Admittance to camp will not permited otherwise. Note that this form is valid for 3 years from the date of the last examination. It is also acceptable to submit a copy of the players school health form containing the same information and signed by a physician.
2. The AC CALDO camp medical director is permitted and trained to administer inhalers and epi pens that campers bring to camp. If this is a requirement of any camper, the parent must submit the [Authorization of Adminstration of Medication Form] prior to or at the start of Camp. Note that this form must be signed by both a parent and the campers physician. Parents must bring the Epi pen or inhaler to camp and it will be held in the camp medical lock box until needed.
